Strict Medication Adherence – p25120059

Skilled nursing visit included education to the patient and caregiver on the importance of strict medication adherence to support effective management of breast and lung cancer. Instruction emphasized taking medications exactly as prescribed and not discontinuing or altering doses without provider direction.

Education was provided on monitoring for changes in condition or symptoms that require prompt medical attention, including new or worsening pain, swelling, unexplained weight loss, increased fatigue, or other concerning signs that may indicate disease progression or treatment-related complications.

The patient was instructed on strategies to manage anxiety related to chronic illness and treatment, including deep breathing exercises, relaxation techniques, pacing activities, and seeking emotional support from family members and healthcare professionals as needed.

Instruction included the importance of attending scheduled follow-up appointments and completing ordered laboratory testing to monitor disease status and treatment effectiveness. The patient and caregiver were also educated on recognizing and promptly reporting any medication side effects or adverse reactions to the healthcare provider or home health agency.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the patient and caregiver verbalizing understanding of medication adherence, reportable symptoms, anxiety management strategies, and the need for ongoing monitoring and follow-up. No barriers to learning were identified at this time. Continued skilled nursing education and monitoring are required due to complex oncologic diagnoses and treatment regimen.
