Skilled nursing visit included education to the patient on the importance of regularly monitoring blood pressure and accurately recording readings to share with her healthcare provider to support effective management. The patient was instructed on recognizing signs and symptoms requiring immediate medical attention, including severe headache, chest pain, blurred vision, dizziness, or markedly elevated blood pressure readings.

Education was provided on lifestyle modifications to support blood pressure control, including adherence to a heart-healthy diet, limiting excess salt and caffeine intake, maintaining adequate hydration, engaging in regular physical activity as tolerated, and implementing stress-management techniques. The patient was advised to notify the healthcare provider of any persistent or worsening symptoms.

The importance of attending follow-up appointments and adhering to all provider-ordered treatments and recommendations was emphasized to promote safety and prevent complications related to uncontrolled hypertension.

The patient actively participated in the teaching session. Teach-back was demonstrated, with the patient accurately verbalizing proper blood pressure monitoring techniques, reportable signs and symptoms, and lifestyle strategies to support blood pressure control. No barriers to learning were identified at this time. Continued skilled nursing education and monitoring are indicated to support ongoing management and reduce risk of complications.
