Fall Risk Evaluation and Home Safety Teachings – p26010079

SN completed fall risk assessment and evaluated the home environment for safety concerns contributing to fall risk. Patient was instructed on safe ambulation techniques, including walking at a steady pace, avoiding sudden turns, and using assistive devices (e.g., cane or walker) properly and consistently as ordered.

Teaching included the importance of wearing non-skid footwear, maintaining clear pathways, removing trip hazards such as loose rugs or clutter, and ensuring rooms and hallways have adequate lighting—especially during nighttime hours. Patient was also advised to keep frequently used items within easy reach to avoid unnecessary bending or overreaching.

SN reinforced the importance of rising slowly from bed or chairs to prevent orthostatic dizziness and instructed patient to pause after sitting up before standing. Patient encouraged to ask for assistance when feeling unsteady or fatigued.

Patient was asked to explain at least two strategies they will use to prevent falls and describe how they will safely ambulate within the home. Patient correctly stated that they will use their walker at all times, avoid rugs, and keep hallways clear. Minimal prompting was required.

Skilled nursing remains necessary to assess safety risks in the home, reinforce fall prevention strategies, monitor mobility status, and coordinate care with PT and the healthcare provider to reduce fall risk and support safe aging in place.
