Skilled nursing visit included education to the patient and primary caregiver on the potential complications of uncontrolled or untreated diabetes. Instruction included discussion of long-term risks such as cardiovascular disease, kidney damage, and peripheral neuropathy. The patient and caregiver were educated that poor glycemic control may also lead to vision impairment, including diabetic retinopathy, and increases the risk for foot ulcers and infections that may require surgical intervention if not promptly addressed.

Education was provided on the risk of hyperglycemic crisis related to unmanaged diabetes and the need for immediate medical attention if signs and symptoms occur. The patient was instructed on the importance of maintaining blood glucose within target range to prevent both acute and long-term complications and to promote overall health.

Emphasis was placed on adherence to prescribed medications, diabetic diet, and lifestyle modifications, as well as the importance of regular blood glucose monitoring and routine follow-up with healthcare providers to support safe and effective disease management.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the patient and caregiver accurately verbalizing potential complications of diabetes, the importance of glycemic control, and the need for medication adherence, lifestyle modification, and ongoing monitoring. No barriers to learning were identified at this time. Continued skilled nursing education and monitoring are indicated due to chronic disease complexity and risk for complications.
