CHF Daily Weight Monitoring: How to Prevent Fluid Retention and Avoid Hospital Readmission – p26040068

Health teachings were provided regarding Congestive Heart Failure (CHF) and importance of daily weight monitoring. Patient was educated that sudden weight gain may indicate fluid retention and worsening heart failure. Patient was instructed to weigh himself daily, at same time each morning after using restroom, before eating, wearing similar clothing, and using same scale for accuracy. Patient was instructed to record weight in a daily log and bring log to follow-up appointments for review.

Patient was advised to notify PCP promptly for any of following:

  • Weight gain of 2–3 pounds in 24 hours or 5 pounds in 1 week.
  • Increased swelling to legs/feet/abdomen.
  • Increased shortness of breath, decreased activity tolerance, or fatigue.
  • New or worsening cough, chest discomfort, or need to sleep upright.
  • Decreased urine output or other signs of fluid retention.

Education included purpose of daily weight monitoring to support early identification of fluid retention, assist PCP with medication and treatment adjustments, and reduce risk for complications and Hospital readmission. Patient was encouraged to follow CHF plan of care and to call 911 for severe shortness of breath, chest pain, or any emergency symptoms.

Patient was able to verbalize understanding of CHF weight monitoring education. Patient stated he will weigh himself every morning using same scale and record results in a log. Patient correctly repeated reportable findings, including weight gain of 2–3 pounds in 24 hours or 5 pounds in 1 week, and stated he will contact PCP promptly if symptoms occur.
