Medication Teaching (Variation 1) – Purpose, Schedule, and Adherence Basics

Use this when: first visit, new meds, patient forgets doses.

Teaching Script:
SN instructed patient and caregiver on safe medication adherence. Education included taking medications exactly as prescribed, using a consistent daily schedule, and not skipping or doubling doses. Reviewed keeping an updated medication list, bringing it to all appointments, and using a pill organizer or phone reminders to improve adherence. Patient advised to contact PCP before stopping any medication, even if feeling better.

Teach-Back Prompts:

  • “Tell me what time you will take your morning and evening medications.”
  • “What will you do if you miss a dose?”
  • “Why is it important not to stop a medication without calling your PCP?”

Red Flags:
Notify provider for persistent dizziness, allergic reaction symptoms, or new/worsening symptoms after starting medication. Call 911 for facial swelling, trouble breathing, chest pain, or fainting.

EMR Copy/Paste Block:
SN provided skilled teaching on medication adherence including purpose of medications, safe scheduling, and missed-dose precautions. Patient instructed to take meds as prescribed, avoid doubling doses, and maintain updated medication list. Teach-back used; patient verbalized dosing schedule and actions for missed dose.
