Skilled nursing visit included education to patient and caregiver on the importance of administering medications at the same time each day to maintain therapeutic effectiveness and reduce the risk of missed or duplicated doses. Education was provided on safe medication management, including the caregiver’s role in direct supervision and assistance with medication administration due to the patient’s recent dementia diagnosis and forgetfulness.

The patient and caregiver were educated on fall prevention strategies, including close monitoring, assistance with ADLs and IADLs, and maintaining a safe home environment to reduce injury risk. Emphasis was placed on promptly reporting any changes in mental status, new or unexplained bruising, falls, or medication-related concerns to the home health team to support patient safety and continuity of care.

Skilled nursing intervention included filling the patient’s pill organizer for the entire week in accordance with the current medication list. Medications were reviewed for accuracy and compliance at the time of the visit.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the caregiver verbalizing understanding of medication timing, supervision responsibilities, fall prevention measures, and reportable signs and symptoms. Patient participation was limited due to cognitive impairment. No additional barriers to learning were identified. Continued skilled nursing oversight and reinforcement are required due to cognitive decline and high fall risk.
