Skilled nursing visit included education to the patient and caregiver on strategies to manage Fatigue related to ongoing Radiation therapy. Instruction emphasized balancing adequate rest with light activity as tolerated to conserve energy and maintain functional ability. The patient was educated on prioritizing daily tasks and breaking activities into smaller, manageable steps to reduce physical strain and prevent overexertion.

Education was provided on the importance of maintaining adequate nutrition and hydration, including consuming small, frequent meals, to support energy levels during treatment. The skilled nurse discussed monitoring for signs and symptoms of worsening fatigue or complications related to the patient’s multiple comorbidities, including lung cancer, congestive heart failure, chronic obstructive pulmonary disease, and atrial fibrillation.

The patient was instructed on proper use of supplemental oxygen as prescribed and advised to promptly report increased shortness of breath, dizziness, chest discomfort, palpitations, or decreased activity tolerance to the healthcare provider or home health agency. The patient and caregiver were encouraged to communicate any concerns or questions regarding fatigue management to the skilled nurse or primary care provider to ensure safe and effective care.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the patient and caregiver verbalizing understanding of fatigue management strategies, energy conservation techniques, oxygen use, and reportable signs and symptoms. No barriers to learning were identified at this time. Continued skilled nursing education and monitoring are required due to complex medical conditions and ongoing cancer treatment.
