Chronic Disease Management and Prediabetes Diet Teaching for Patient and Caregiver – p26010075

During a home health visit, the skilled nurse provided ongoing education to patient and caregiver on chronic disease management, safe medication use, signs of infection, blood pressure monitoring, fall prevention, and infection control, including COVID‑19 screening. Teaching covered correct home blood pressure technique, how to recognize signs of infection or worsening condition, and when to contact the healthcare provider or call 911. The nurse also assessed fall risk and home safety, reviewed safe ambulation and proper use of assistive devices, and suggested simple environmental changes to reduce fall risk.

Because the patient was prediabetic, the nurse provided dietary guidance focused on a diabetic style diet, including balanced meals, portion control, limiting sugary drinks and refined carbohydrates, and avoiding skipped meals. The patient and caregiver repeated the key points in their own words, demonstrated correct BP monitoring, and identified specific infection warning signs and fall prevention strategies, showing improved understanding and self management skills.
