SN visit included education to the patient and caregiver on proper wound care techniques, including hand hygiene and correct dressing change procedures, to promote optimal wound healing and prevent infection. Instruction was provided on monitoring the wound for signs and symptoms of infection, including redness, swelling, increased drainage, warmth, or foul odor, and the importance of reporting these findings promptly to the physician or home health agency.

The patient was educated on blood glucose monitoring and the importance of maintaining target blood glucose levels to support wound healing and overall health. Instruction included adherence to prescribed diabetic regimen, monitoring frequency, and recognition of abnormal blood sugar levels.

Due to lower extremity weakness and edema, the patient received education on safe ambulation techniques and fall prevention strategies, including proper use of assistive devices, slow position changes, environmental safety, and elevating lower extremities as appropriate.

Patient and caregiver actively participated in the teaching session. Teach-back was successfully demonstrated, with the patient and caregiver verbalizing understanding of wound care procedures, signs of infection to report, blood glucose management, and fall prevention strategies. No barriers to learning were identified at this time. Continued reinforcement will be provided as needed.
