Fall Prevention Strategy – p25120053

Skilled nursing visit included education to the patient and caregiver on implementing fall prevention strategies to reduce injury risk. Instruction included removing tripping hazards, ensuring adequate lighting throughout the home, and proper use of assistive devices as needed. The caregiver was instructed to provide close supervision during transfers and ambulation, particularly when the patient appears drowsy or fatigued.

Education was provided on the importance of adhering to recommended sleep apnea management, including following up on sleep study results and consistent use of prescribed therapies as ordered. The patient and caregiver were instructed on recognizing signs and symptoms of excessive sleep deprivation, including confusion, poor balance, increased daytime drowsiness, and elevated fall risk.

Emphasis was placed on promptly reporting any falls, near falls, worsening drowsiness, or changes in mental status to the physician or home health agency to prevent injury and complications.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the patient and caregiver accurately verbalizing fall prevention measures, the need for close supervision during ambulation, and signs and symptoms related to sleep deprivation and sleep apnea that require reporting. Patient and caregiver verbalized understanding. No barriers to learning were identified. Continued skilled education and reinforcement will be provided as indicated.
