Skilled nursing visit included education to the patient and caregiver on energy conservation strategies to minimize respiratory distress, including pacing daily activities and incorporating frequent rest periods to prevent exacerbation of shortness of breath.

Education was provided on the correct use of prescribed respiratory medications, including inhalers and nebulizers, to optimize airway function and symptom control. Proper technique, timing, and adherence were emphasized. The patient was instructed on regularly monitoring oxygen saturation, documenting readings, and sharing results with the healthcare provider as directed.

The patient and caregiver were instructed on recognizing signs and symptoms of respiratory distress, including increased shortness of breath, wheezing, chest tightness, cyanosis, or decreased oxygen saturation, and advised to seek immediate medical attention if these occur.

Education also included the importance of maintaining adequate hydration and nutrition, as well as ensuring a safe home environment to reduce fall risk and prevent complications related to decreased activity tolerance. The patient and caregiver were encouraged to promptly report any changes in respiratory status or overall condition to the skilled nurse or primary care provider to support safe and effective management of COPD.

Patient and caregiver actively participated in the teaching session. Teach-back was demonstrated, with the patient and caregiver accurately verbalizing energy conservation techniques, proper use of respiratory medications, oxygen saturation monitoring, and reportable signs and symptoms. No barriers to learning were identified at this time. Continued skilled nursing education and monitoring are required due to chronic respiratory disease and risk for exacerbation.
