SN instructed patient that rising slowly from a sitting or lying position can help prevent dizziness or lightheadedness. Staying well-hydrated, avoiding alcohol, and eating smaller, low-carbohydrate meals that include fruits and vegetables may also improve symptoms.

Patient was advised to seek medical attention immediately if any of the following occur:

  • Fainting.
  • Breaking out in cold sweats.
  • Rapid or shallow breathing.
  • Presence of blood in stool.

SN provided and reinforced education regarding disease process – hypertension, and signs and symptoms to report. Patient verbalized understanding.
